Fees & Forms for Port Ludlow Counseling
Payment is due upon receipt of service. Each 60 minute session for individuals is $100. Couples therapy is $150 per 60 minute session.
I accept cash, check* or credit card. I do not bill insurance directly, however I can provide a statement you may submit to your insurance company for reimbursement. I also offer a sliding scale for special circumstances; please discuss your financial need with me.
I will email you a Client Information Sheet prior to your first session. Please print and fill out the Client Information Sheet prior to your first appointment and bring it with you. Thank you.
*Any returned check will be charged an additional $25 processing fee
Therapy appointments are approximately 50 minutes in length. There is no charge for appointments canceled within 24 hours.
Should you need emergency assistance please call the crisis line in Jefferson County at (360) 385-0321. In Kitsap County at (360) 479-3033. In the event of a medical emergency please call 911.
You have the right to confidentiality. Your privacy is very important to me. The client-therapist communication is considered strictly confidential and privileged information. There are a few legal exceptions to this that I have outlined in the Disclosure Consent Form you sign prior to beginning therapy. This form will be given to you and discussed in greater detail at our first appointment.
